There is online survey tool called LimeSurvey. It is open sourced and depends on PHP and MySQL (if you have website, then I believe you do meet these requirements). It gives users option to save survey and resume for later. It is used by quite a few universities out there. I recommend you give it a try. Their website is at http://www.limesurvey.org/ .
