I asked something similar a few weeks ago and got a somewhat cryptic answer 
which won't work for me because I don't have permission to issue directives on 
the machine involved which uses Apache 1.3.37  but. . .

It appears that apache checks for extra filename extensions that are keyed to 
MIME-like types that you have to allow for with directives.

I'm still looking for something that will honor my html code lines like

HTTP-equiv: type text/html; charset utf-8

but I haven't found it yet. My web host is about to do a major update and 
things might soon be different.
