Something like that:

1. Configure an AJP connector in your Tomcat. If you used mod_jk you
probably already have one.

2. Add proxy directives to your httpd.conf:

   ProxyPass /app ajp://TOMCAT_HOST:AJP_PORT/app
   ProxyPassReverse /app ajp://TOMCAT_HOST:AJP_PORT/app
