Apache 2.4 splits the "remote_ip" into a client IP (TCP peer) and a
useragent_ip (e.g. X-Forwarded-For)

Arbitrarily choosing the former for mod_cband lets it build:

... and for very simple tasks you might want to have a look at mod_ratelimit, which is bundled with 2.4. Improvements welcome ;)
