The path is not the problem:

I tried all possible paths - relative, full path, windows path. I also tried to 
copy the video to the PHP code's directory so the video file and the PHP code 
were in the same place and no path was needed.  Also this same PHP code works 
fine with other video player.


In the past I had the same problem when I was trying to use QuickTime player. 
Finally I somehow made it work using some change in htaccess, but I don't 
remember what I did. It was long ago and I already deleted that version since I 
decided not to use QuickTime.

Please advise.

Have you checked paths (local vs. relative, plugin location, etc.) in
the html file?
