I just found

which suggests:

    RewriteEngine On
    RewriteCond %{LA-U:REMOTE_USER} (.+)
    RewriteRule . - [E=RU:%1]
    RequestHeader set X-Remote-User "%{RU}e" env=RU

And that exactly does the trick.
