I see....you have a onreply_route[handle_nat] in your script, doing:

    if ( isflagset(NAT) )
        rtpproxy_answer("ro");

Change that to :

    if ( isflagset(NAT) && has_body("application/sdp") )
        rtpproxy_answer("ro");
