OK, I now have a document with:

Page 1:
"Heading 1" at the top, formatted as a heading - no title on page 1

Page 2:
"Heading 1" in a frame (looks just like a header) at the top, formatted as a heading - this will be whatever "Heading 1" on page 1 is (referenced)
"Heading 2" at the top of the text area, formatted as a heading


Page 3:
"Heading 2" in a frame (looks just like a header) at the top, formatted as a heading - this will be whatever "Heading 2" on page 2 is (referenced)
"Heading 3" at the top of the text area, formatted as a heading
... and so it goes.
This requires setting each page reference individually, though I'm sure a macro could be written (not really my field)
