Right click in the tool bar area that you want to add the button to and select 'customise'. In the 'available button' section scroll down to 'view'. Expand that section and look for the item titled "page preview". Select it and then click "add" button to add it to the existing icon set.
You can also change where it appears in the tool bar by dragging it up and down once it is in the 'buttons in use' area. Have fun.
