Ah, good catch! I tested the same and when formated for two columns it
works with page size letter. To test I put borders on the frames and
printed against a WorldLabel (which uses tables) and then compared
against a strong light - they matched exactly.

I would have to say that a template for the labels should be on the correct page size. That said I still love OO and will just use3 my work around until this issue is resolved.

 I reckon that someone should file a bug. Search in the bug database
shows some issues back in 2003, but nothing recent that I can find.

Can you not change the "User" by selecting 5162?

To see if your settings are OK under Writer, select File|New|Labels and set:

Brand: Avery Letter Size
Type:  5162 Address

That should properly modify to
5162 Address: 4.00" x 1.33" (2 x 7)

If you click on the 'Format' tab, you'll see Horizontal pitch, etc
infomation that you can use to manually modify a page if necessary. Now
click on the button to create a new page.

On mine Format|Page does show:
Format: User
Width: 8.55"
Height: 10.18"

And prints to my Printer (Canon MP750) just fine. However, you can
change to 8.50" via Format|Page by selecting:
Format: Letter
if necessary. But that will then change the template to two pages (not
helpful).  Alternately, you can use Worldlabels & their template:

Worldlabel are the only label maker that I know of that include OOo
templates via their website for their labels.
The WorldLabel wl-ol100 properly uses 8.50 "x 11.00" Letter as default
in their template.
