SOLVED.  It had to be added to styles that appear in the index.
Right click on the index>Edit Index/table
check the box of Additional styles and click the button (with the three
dots) in front of it
select the style you want to appear from the list, shift it using the button
(with the arrow pointing left) to the required indentation, press OK and
your new style will appear in the index.
