If you are old enough, you might remember printed-on-paper technical
books... :-)
Some of them had color blocks printed right to the outside edge of the
pages, so that they'd show (vaguely) when you viewed the closed book
from the side, or when you had the book open with a mass of pages on one
side of the spine. The color blocks were maybe a centimeter in height,
and their position changed from chapter to chapter. So, viewed from the
edge of the closed book, you'd see staggered steps from the beginning of
the book to end.
The method you describe is indeed still done for some books.
Presuming that each chapter has a separate header, it should be possible
to create a thumb-tab for that header as a frame, anchor the frame in
the header but place it outside at the position that you want it to
print. You can set the background color for the frame to any color you
want and place any text you want in the frame.
However, very few printers will allow you to print right up to the edge
of the paper. Unless you are trimming after you print, you are likely to
encounter problems in that area.
