A simple "touch /var/cache/pkg-cacher/exlock" did also fix this problem.
Right now it's downloading the packages, so everything seems to work!
I think it would be a good idea to fix these to issues in the debian
packages:
1. Make procmail a dependency.
The lockfile binary code was inherited from the original apt-cacher. I
didn't realize it was supplied by procmail, thanks for pointing that out.
2. Create /var/cache/pkg-cacher/exlock.
The script is supposed to do this automatically. I'll look into this.
This would probably save others some time.
