Since it's been a bit quiet in this thread I've added a couple of comments
to the code review, carefully picking the low-hanging fruit of the
initialisation issues and avoiding the awkward test failure :)
As per review comments, I hope I have contributed more than just stating
the obvious and apologies if I have not.
