I experienced this problem in conjunction with other problems since
upgrading to 3.2.2 with cPanel 11, Exim 4.66, Perl 5.8.8. Other problems
included not recognizing user_prefs and not rewriting the subject properly.
Temporarily resolved all issues above by disabling Razor, Pyzor, DCC and
restarting Exim.
