Try seeing if you can find an old copy of the ISAPI redirect installer. If you can't find one by google, let me know. I had a heck of a time with getting IIS 5 and the redirect to work. The installer for whatever reason seemed to do the trick for me.
Also, make sure that you physically restart your machine (if possible) after configuration. That was the only method that worked for me.
