My first guess would be that the manager app is not running.

I guess that maybe your manager.xml or
webapps/manager/META-INF/context.xml is broken
(like a typo somewhere: a stray '>' or a nested comment ('--' is not
allowed inside comments)). There should be errors logged somewhere.
