I wrote an jira issue about attachments problem on last Glassfish version 3.1.2 ... This bug related only with glassfish. For example, if you working with XWiki 3.4 and old glassfish - attachments works, after updating Glassfish to 3.1.2 (without any other changes) - stops working. http://jira.xwiki.org/browse/XWIKI-7674
