The paper format is determined when the PDF document is created -- with Acrobat Distiller.
One of my "printers" is, in fact, the Acrobat Distiller printer driver, which creates the PDF document directly from Word 2000, Publisher 2000, or any other similar program. As with other printer drivers, I can choose the paper size and orientation.
