It's not printed on A4 -- that is the finished size. It is printed two-up on 19x25" sheets (a fairly common size used by printers) then trimmed to A4.

FWIW, very little printed material (such as books, magazines, almost anything in color) is printed on final-size paper. It is almost always printed on oversize paper, often numerous copies per sheet (two-up, four-up, etc.), then trimmed to size using a shear or steel-rule die.
 
How many CMS/CAMS are there?

I don't know the answer, but can tell you that my company alone has about a dozen CMS/CAMS employees, plus another dozen with our lower-level, in-house QMS certification.
