When I run it without the "ADD" block, it streams fine with both the channels streaming.

This is VERY unlikely to be a USRP problem, per se. Best to discuss this on the discuss-gnuradio mailing list.

I have two-channel B210 flow-graphs that have been running for months without a problem.
