As mentioned, the Perl module, PDF::API2 is excellent.  In fact, the new 
Programming Perl Graphics book by O'Reilly (ok, so I haven't finished my plug 
review writeup still) has an example specifically doing what you are asking.  
automated mailing lables :)

Sounds like somehting you should check out.
