There's a local company whose name I've forgotten that offers local Utah
DID numbers that they will terminate at your asterisk server for a
pretty good price.

But for me google Voice provides the number and eliminates most of the
need.  If you want to dial out with Google voice there are recipes for
operating GV through http and getting it to call back into my gizmo sip
number.
