Hi, I've seen this happen only on Windows, and usually when you're running VirtualBox as another user already. VirtualBox on Windows has all sorts of problems if you run it as an admin and then try to access it as a user (or vice versa). Please make sure you're launching Vagrant and using VirtualBox with the same user.
