Check the error codes on the robot if you can.
Check how many times the tape has been mounted.
Check the S.M.A.R.T. read and write errors on the tape drive.
Check for errors in $NETBACKUP/db/error for that tape ID.
Check how long the tape drive has gone without cleaning (tpclean -L)
