I've put the patch into source control, improved the appearance of the 
dialog box a little, and changed the default value for the ignore text 
option. It now remembers what setting you had previously (along with the 
prefix and suffix options). If you update from svn or bzr you should get 
this.

Do you want the Makefile in upstream or other files? I thought debian 
don't like upstream to have a debian directory. (referenced here 
http://kitenet.net/~joey/blog/entry/on_debian_directories_in_upstream_tarballs/ 
) Should it be renamed as something else?

Unfortunately distutils doesn't seem to do an uninstall.
