If someone knows about this ioctl to set/read gpio pins directly, could 
you please post it? I have a PC/104+ bt878 card (it's called a Sensoray 
311) that has some of the gpio pins just routed to a header for doing 
generic digital io (which could be used for camera triggering, for 
example). It would be fun to try it out.

As for the 16-camera multiplexer, it may be easy to figure it out if you 
can find out which chip the mux is, and how it's connected to the bt848. 
 It seems to me that the way they'd do it would be to switch 4 lines at 
a time, since the 848 has a 4-way mux internal to it.
