Cat has worked for me also, only it gets the time coding wrong (visible 
from the DVD player). I use mpeg3cat which is part of the libmpeg3 pakage.

Unfortunately, it doesn't work directly on system streams. You need to 
concatenate the video and audio streams separately and then multiplex 
them together. I think mpeg3cat is capable of extracting the audio and 
video streams. I use mplex to multiplex (mjpegtools).
