I cannot answer for Bram, but my own philosophy is "find a way to make it work". If this is not very linear, well, that's life. Neovim tries to address your concerns. I don't really know how it works, but you could have a look at it. I am happy with the *status quo* in Vim.
About the help files, there is a reason for all that. The English tags file is named "tags". Every "national" file is named tags-ll. Easy to find and easy to check. The READMEll.txt file is not really necessary, but it explains something about the way the translation was managed. For Italian, it also states the patchlevel of the translation. The .llx files are the real thing and they can coexist with the .txt files in the doc directory. It seems rather logical to me (maybe we don't have the same perspective on the matter). In my case, when I update a Vim help file, I just call a script (e.g.: updaitx options). All the rest (managing tags, READMEit.txt) is buried in the script.
