Just FYI...

After not having used MacVim for a while, the ESC key would no longer work to 
exit Insert Mode; I did find that <control>ESC did work.  It only occurred for 
my ID.  As it turned out, I had tried turning on Accessibility's "Speakable 
Items" to try it out after upgrading to OS X Mavericks in June, but then forgot 
about it.  The default key to turn "Listening" on/off is ESC, which was 
overriding it everywhere.  So, if you want to use "Speakable Items", you can 
either use <control>ESC in place of just ESC or you can switch to use a 
different "Listening" key in:

Apple ->
     System Preferences ->
          Accessibility ->
               Speakable Items ->
                    Listening Key ->
                         Change Key
