I just figured it out by going through some Service Source manuals. I
was thinking about what other standard interfaces besides Mac video
use DA-15 connectors: AUI and PC joystick port was all I could think
of, and neither seemed likely to be found on a Mac.

It turns out it is a PC joystick port after all, and that's the card's
sole purpose. It came in LC/Quadra 630 and Performa 640 DOS Compatible
models. The internal connector attaches via a short cable to the 486
daughterboard which is piggybacked onto the Mac's main board. The
skinny shape is to get around the daughterboard; any other LC card
wouldn't fit.
