I am trying to reconstruct a label from what appears to be the
image using shots of elements scanned from inside the book.
I was lucky to get a good condition book as well but no disk was
included. When I come to pass on the book, I'd like to include a
realistic looking disk in the little plastic envelope!

Hi Keith,
Check out the following link to the scan of the label I have with my
book (I plan to "rebuild my disc" with the link for the software that
was posted, disc is not readable:-( ). Careful 1.9 MB in size. Let
me know if you have issues accessing it I can rename it if needed.
