I assume you know how to create ringtones.  You can go to:

In the edit box, enter this Youtube Link:

Now, when it converts the ringtone to an mp3, save it to your computer.  You 
can use one of the common conversion programs to convert it to an .m4R file. 
The editor I use converts it to .m4A and then I just rename the extension to 
.m4r and put it in my ringtone folder.
