If you go into settings, general, blue tooth, do you see a list of
your blue tooth devices? If you doo, flick to the more info button
next to the device you want the phone to forget. I believe that is
where the remove from device option may be found.

Try removing it from the device and re-pairing it. That worked for me
when my 4s lost my Bose earphone.
