When this is intermittent, it often results from one or the other phone
falling off the network.  Sometimes the drop is so brief that it is not
easily detected.  A recent contact with AT&T regarding a phone with this
behavior yielded such helpful responses as:
They do that sometimes.
In short, they don't really know.  In this case the person involved does
not have a texting plan.  So, when this occurs, she gets charged for a text.
