I have noticed this problem on many apps since the 8.1.2 update. What appears to be happening is that the focus moves away from the editing field. When this happens, I have to just double-tap into the edit field again, voice over will say "Cursor at end of text", then the keyboard appears again.
I have noticed this problem is caused when I click anywhere not on the keyboard when in an edit field, the focus moves much like voice over is turned off and a single tap will move focus. I have to be extremely careful not to touch anywhere off the keyboard when entering text. HTH!
