I wanted to add something not mentioned in the procedure outlined at the link I posted about asking for app refund. THe easiest way I found to get to the "report a problem" link for requesting a refund of an app can be found in the receipt mail one gets after buying the app. One more great reason to make sure to keep these emails safe. In that mail there's a link titled "report a problem" and you just have to sign in with your apple id and password and you land in the form where you can indicate the type of problem you are having and submit your request. OF course, I don't think one can over use this too often. But, as long as everyone uses this great feature in good faith it might actually sharpen awareness about inaccessible apps both at apple and the developers.
