The apple TV has an activation code that it gives you, and you need to go to a specific website to activate it. The instructions are displayed and read with VoiceOver though, so it’s not hard to do. As far as the iOS app goes, you can purchase the subscription through that, which you can also restore to your apple TV if the code is giving you trouble.
