Not sure if anybody else is experiencing this issue, but once in the pass code for the iPhone and having the passcode turned on, VoiceOver will immediately take focus to the emergency call button instead of allowing the user to enter their pass code.
I have read and other groups related to accessibility that this is an issue with iOS 5 as well as voiceover I'm hoping that the accessibility team Apple will have this fixed when iOS six comes out. --
