Yes, these things are very different. What I would do would be to explain to 
the developer that VoiceOver, when it is enabled, modifies the gestures one 
would normally use on the iPhone. For example, you would normally just tap one 
on something to activate it. With VO enabled, this gesture is accomplished with 
a double tap. A tap and hold for a sighted person is the same as a double tap 
and hold with VO. Scrolling for a sighted person is done with one finger. With 
VO, it is done with three fingers.
