I've noticed this issue going back to iOS 8.x. triple clicking the home button 
to turn VO off and doing the same to turn it back on fixes it. Cant' do it too 
quickly otherwise the skipping will still be there, but waiting a couple of 
seconds is usually enough time. It'll happen more often than not after ending a 
phone call, be it making or receiving one, but not always.
