Hi Candy, I put 2 links below, I found them easily enough with Google, but there don't seem to be a ton of apps that do this. I can't speak to the accessibility except where Voiceover support was mentioned in the description, but some of these are free apps so nothing lost if you try them.
My MP3: Free https://itunes.apple.com/us/app/mymp3-convert-videos-to-mp3/id834818663?mt=8 ProTube for YouTube: $3.99, mentions "full Voiceover support" in the app description This app I think doesn't allow for conversion to MP3 and offline playback, I read the developer had to remove this as it violated Youtube terms of service. However, it does allow you to play back videos in 256 Kbps quality which of course takes a lot less bandwidth than playing video and it allows for background playback with the phone locked.
