Yes, you hear the effect when it's sent and the recipient hears/sees it if they 
are not running Voiceover. But as far as I can tell you do not get any 
visual/audio effects if you receive a message with said effect and if Voiceover 
is on. In this case you just gett he description.

I could hear the fireworks and lasers after I send them and I have confirmation 
fireworks could be heard on the other side. I also hear VO description. Maybe 
this is because I don't use Audio Ducking.

Voiceover is running the effects don't actually play when a message is received 
but instead you get the description as Chris described. I was sending messages 
to my wife who was right next to me and I still had Voiceover on and was 
wondering why the fireworks and so on didn't play, then as soon as we turned 
off Voiceover all was as it should be.
It also seems that once the message is received that if you go back to it in 
the conversation it no longers has the effect description.

Lastly, my wife sent me a message where she used handwriting as I was curious 
as to what Voiceover would do with that. She just wrote "Hi" and Voiceover said 
the message had handwriting and "2 strokes".
I then tried it and wrote something and Voiceover said it had 9 strokes, but of 
course there is no way for Voiceover to figure out what it says which is 
expected.

The way I tested it, and to see what it is like, is I sent messages to myself.  
I know, boring, but it works!! :)

To get message effects to work, just do a double tap and hold on the send 
button and a pop up window will appear.  Now there is no sound with the pop up 
window, and sometimes voice over does not focus on it, but if you explore the 
screen a little, then try swiping left or right, you will find yourself in the 
effects window.
Now there is two types of effects, bubbles and screen.  Bubbles are selected by 
default.  If you land on bubbles, you will notice that voice over says, bubbles 
selected, one of two.
Now there is three or four different bubbles to choose from, Slam, loud, 
gentle, and one more that I can not remember, smiles!  I sent myself a test 
message with a slam bubble effect, when the message came in, voice over said 
something like, a message from Chris comes slamming in with dust flying up 
around it.  So voice over does describe the effect, so that is pretty cool!!

Now, in the pop up window, if you swipe one time past bubbles, you will hear 
screen.  This is the second type of effect, so just double tap it to bring up 
the screen choices.
Now if you swipe right you will come to the first of five screen choices, 
balloons.  This is a button, so just double tap it to send the balloon effect 
with your message.
If you swipe past balloons, first is a close button, then you will find a 
counter, and it will say, one of five.  To move between the different screen 
effects, just flick up or down with one finger.
As screen effects you have, balloons, confetti, lasers, fireworks, and shooting 
star.

Now I know these effects might not work perfect, but since they are very 
graphical in nature, I think it is pretty cool that not only can we send the 
effects, but voice over also describes the effects when the message comes in.  
Thank you Apple for including accessibility!
Now I will only mention one more thing.

When you receive a message, if you flick up or down with one finger on the 
message, within the choices will be react.  If you double tap on react, it lets 
you send a reply back with your reaction to the message.  When you double tap 
on react, a pop up menu will appear with different reaction choices like, red 
heart for love the message, thumbs up, thumbs down, and more.  To send your 
reaction, all you have to do is to double tap on your choice and it is 
automatically sent.
Another cool thing!!
