I know exactly what volume problem you're talking about. It has nothing to do with the volume settings of either Siri or voice over. Voiceover get super loud during phone calls or when the alarm is going off. Then once the other activity is done, VoiceOver goes back to its normal volume. I reported this bug repeatedly but got no response and it's very annoying.
