Larry, I have experienced this randomly when connecting my lightning earbuds or my older headphone jack earbuds which were connected to the new lightning dongle to my phone. The few times it has happened, I unplugged the earbuds and then plugged them back into the phone and they worked just fine. It has not happened a lot or often enough to be a pattern.
