It works now that I rebooted the device. Others have said that when this happens to switch VoiceOver off and then back on again instead of always rebooting. I love it, I am using it with my 6th gen iPod and my Apple TV 4th gen. May try it with my PC too.

The only time I have ever seen that was when the batteries were going dead. A fresh set eliminated the issue.
