If she cannot do the rotor, the first thing I would have her do is go into the 
VoiceOver settings, Audio, and turn on audio ducking.
That way, other audio volumes will be lowered while VoiceOver is talking.  That 
may help a little.
Otherwise, other than the medthod you described by starting VoiceOver reading 
and hitting the up volume button is the best way to turn it up.
Or, have someone else check that Volume is in the rotor, have them turn the 
rotor to volume and flick up with one finger until it is at 100%.  You may get 
some distortion at that point, but she could see if that helps.
Or, if she has a Bluetooth keyboard, the rotor can be done by pressing the 
right arrow plus the up arrow to turn it clockwise, or left arrow plus up arrow 
to turn counter clockwise.

I have a friend who is having difficulty getting voice over turned up, and she
