You're right. I believe this is a new problem introduced in, perhaps, IOS 10. I don't ever recall this happening before the past several months.
The solution that works for me is to move my finger to the dictation button and hold it there and wait until VO speaks the prompt" double tap to start dictation ." Then after this has finished, double tap and dictate. For me double tapping with two fingers seems a less reliable dictation start.
