It will be interesting to see how this works in the real world once these phones are in the hands of blind users. It's nice Apple is thinking of this, but I thought it was neat how Face Id wouldn't unlock a phone if the person had their eyes closed. This makes total sense because if it could be unlocked this way anybody could pick up your phone if you are asleep or what if you had an accident and are unconscious or what if somebody simply hit you on the head because they want to steel your phone. They could then unlock it quite easily by simply pointing it to your face. Of course in the situation where you are out cold because of an accident or because you got bonked on the head somebody could do what they want with your hand as well and unlock using Touch Id. Nobody could pick up my hand to put it on the Touch Id if I'm asleep without waking me up, but you could probably pick up my wife and put her in a tub filled with cold water without waking her up when she's asleep *smile*. Anyhow, I would probably re-enable the attention feature first to see if it works just for security, if it's disabled by default when Voiceover is running and a smart iPhone thief knows this they could simply bring up SIRI, tell SIRI to turn on Voiceover and then this security aspect would be disabled. This means you will not have to have your eyes open I believe. This was mentioned in an applevis post.
