Hello Mark, I actually called Apple Accessibility about this issue in September as I had a similar problem and was told that they could not reproduce my issue. I was recommended to exit the timer app by pressing the digital crown once and reopen it and then set the custom timer again. Once I did this, the start button appeared as available. I was also told to try quitting the app in the switcher and reopen it and if the problem persisted to also reset the watch. I have the series 1.
